Although we appreciate any donation you are able to provide to help us sustain our clinical operations, NO PATIENT will EVER be turned away because of inability to pay.

If you are a patient and need a diagnostic ultrasound procedure, please ask your physician to complete a patient referral form below. Once completed, please email the form to contact@umiclinic.org.

Once we receive the form, the clinic's staff will reach out to you to schedule your appointment.
