UMI Community Imaging Clinic provides physician-ordered ultrasound scanning at no cost to patients.
We perform ultrasound image acquisition, and all exams require a referral from your healthcare provider.

Although we gratefully accept donations to help sustain our clinical operations, no patient will ever be turned away due to inability to pay.

Diagnostic Ultrasound Referral

If you are a patient in need of a diagnostic ultrasound, please ask your physician to complete the referral form linked below. Once the form is completed, please email it to contact@umiclinic.org.

After we receive the referral, our clinic staff will contact you to schedule your appointment.
